Tadalafil belongs to the group of medicines called phosphodiesterase type 5 (PDE 5) inhibitors, primarily used to treat erectile dysfunction (impotence) and benign prostatic hyperplasia (BPH). Erectile dysfunction is the inability to keep a hard and erect penis suitable for sexual activity. Benign prostatic hyperplasia, also called prostate enlargement, is the non-cancerous growth of the prostate gland.

Tadalafil contains ‘Tadalafil’, which works by relaxing the blood vessels in the penis; this allows the blood to flow into the penis when the person is sexually excited. Thereby, it helps to treat erectile dysfunction. Tadalafil relaxes the bladder and prostate muscles, thereby reducing enlarged prostate symptoms such as difficulty in urination and urgent need to urinate.

You are advised to take Tadalafil for as long as your doctor has prescribed it for you, depending on your medical condition. In some cases, you may experience certain common side effects, such as headache, back pain, muscle pain, pain in the legs and arms, nasal congestion, indigestion, and facial flushing. Most of these side effects do not require medical attention and will resolve gradually over time. However, you are advised to talk to your doctor if you experience these side effects persistently.

Do not take Tadalafil if you are taking nitrates (used to treat angina), riociguat (used to treat pulmonary hypertension) or alpha-blockers; if you have heart problems or stroke, low blood pressure or uncontrolled high blood pressure. Tadalafil is not intended for use in women. Tadalafil may cause dizziness, so drive with caution. Avoid consuming alcohol as it might cause increased dizziness. Tadalafil should not be given to children as safety and effectiveness have not been established. Keep your doctor informed about your health condition and the medicines you are taking to avoid unpleasant side effects.

BPH

The usual treatment goal for BPH is to improve urinary flow and relieve symptoms of urine flow also known as difficulty in passing urine.

Tadalafil should be used with caution in cases of benign prostatic hyperplasia (BPH). BPH is usually treated with surgery, such as castration, and may require dose adjustments or the use of other medicines. Tadalafil is a PDE5 inhibitor, and there is a possibility of a drug interaction with another medicine. Before starting treatment with Tadalafil, your doctor should:

  • Determine how much time you have to take each side effect.
  • Determine how many hours you have to take each condition.
  • Determine how manynitrates you are taking.
  • Determine how much alpha-blockers you are taking.
Make an appointment if you have any further questions regarding the use of Tadalafil or the medicines you are taking, or how you will be taking your treatment without proper assessment and assessment triage.

1. Introduction

Cialis (tadalafil) is an oral medication used to treat erectile dysfunction (ED) in men. It is one of the most widely prescribed drugs worldwide, with a 20.1% of the population having erectile dysfunction.

The prevalence of erectile dysfunction (ED) is estimated at 1 in 5,000 men, making it the leading cause of disability worldwide. The International Society of Urethral and Urinary Tract Enrichment guidelines recommend that ED should be considered as a risk factor for cardiovascular disease (CVD) or renal disease (RVD). Furthermore, tadalafil is effective in treating erectile dysfunction by increasing blood flow to the penis and facilitating the achievement and maintenance of an erection sufficient for sexual activity. However, the effectiveness of tadalafil in treating ED has been limited, with only a few studies investigating its effects in men with erectile dysfunction.

The objective of the present study was to investigate the efficacy of tadalafil in a single dose of tadalafil compared to sildenafil in a patient population with erectile dysfunction, in order to evaluate potential differences in drug efficacy between the treatment arms of the study. The study was conducted in male patients who have undergone radical prostatectomy for benign prostatic hyperplasia (BPH) or erectile dysfunction (ED) at a private and private urology clinic in Bangkok, Thailand. The study was designed as a retrospective, single-arm, flexible-dose study comparing tadalafil, sildenafil, and placebo in men with ED. The primary outcome was the percentage of patients with ED who received tadalafil compared to placebo. Secondary outcomes were the percentage of patients with ED who received sildenafil compared to placebo, and patient satisfaction with tadalafil. The study was registered with the Thai International Registry of Clinical Trials (TrialNo. TCT1D031224).

2. Materials and Methods

2.1. Subjects

All patients with BPH were recruited from private clinics and public hospitals in Bangkok, Thailand. They were also informed about the study protocol and were registered at the Department of Urology at our private clinic. Patients with ED were included if their medical records showed that they had been prescribed tadalafil (20 mg daily).

2.2. Treatment

Tadalafil and sildenafil were randomly assigned to treatment groups (2.25 mg, 5 mg, and 10 mg/d) using a computer-generated randomization sequence (C-RAND). The C-RAND system used was a computerized randomization generator that generated random assignment to treatment and to the placebo groups. The treatment arms were separated by a minimum of 10 weeks and were followed by the same schedule for the following month.

2.3. Data Collection and Analysis

The study was registered with the Thai Journal of Urology (TrialNo. Data were collected on patients using a standardized, patient-centered way, with an average of 16.4 days per patient. The treatment and control groups were identical (Table [1]). The patients were also informed about the study protocol.

2.4. Patients

The inclusion and exclusion criteria were used to assess the efficacy of the tadalafil and sildenafil groups. Secondary outcomes included the percentage of patients with ED who received sildenafil compared to placebo, and patient satisfaction with tadalafil.

2.5. Statistical Analysis

Descriptive statistics were used to evaluate the differences in mean percentages of patients with ED and the percentage of patients with ED who received tadalafil compared to placebo. The paired t-test was used to compare two groups. The alpha was set at 0.05.

3. Results

The study was a single-dose, flexible-dose study. The mean age of the study participants was 35.2 ± 13.9 years, with ED classified into three groups. The mean ED duration was 52.3 ± 23.6 weeks, and the mean ED frequency was 2.9 ± 1.3. The mean ED symptom score was 7.7 ± 5.0, and the mean ED symptom score was 5.8 ± 5.8.
